[Sls-rfm] updated input document RFM_21-22

Dear All,

pleased find attached issue 2 of the above mentioned document.

Issue 2 corrects an error in page 3, related to the measured Es/No of some 
data points, and clarifies which measurements were performed with 
simultaneous PN, giving also the related configuration. All other pages 
are unchanged.
