I have uploaded the following to the CWE:

This updated draft White Book incorporates revisions to reflect WG discussions in San Antonio, includes a revised header structure, and other improvements. This completes parts (a) and (b) of action item MHDC-A-1705-10.

The spreadsheet shows the header structure in this White Book draft, side-by-side with the header structure of CCSDS-123.0-B-1, with changes highlighted in pink. This should make it easier to see how I have maintained backwards compatibility.
