[Moims-sc] Updated M&C draft

As promised please find the link for the latest draft of the M&C
specification below:

I have migrated the model and operations to use the approach where no
information is lost when modifying things like check definitions. There
are still approximately 20 RIDs outstanding which I will attempt to work
through next week but I wanted to get this copy out now so that people
have time to work through the changes before Pasadena.
