[Css-csts] Qualified Parameter Examples Annex

At the Fall Workshop, I took the action to produce an informative annex for the SFW that would provide detailed description of the ASN.1 construction of qualified parameters in both the CSRS procedure parameter form and the functional resource parameter form, based on the description that I had provided in email exchanges with the Forward Frame CSTS prototype team.

Attached is my proposed draft of that annex. I created it using the styles in the SFW so (hopefully) it should be able to be inserted directly into the SFW. I have tagged (using Word markup comments) the items that need to be linked to other sections of the SFW. The new annex also references the MD-CSTS and FF-CSTS books, and these should be added to the SFW's  Informative References annex (annex I).

Finally, I suggest that a reference to this new annex be added to the existing annex C, Qualified Parameters. Alternatively, the contents of this new annex could simply be included at the end of annex C in order to keep the Qualified Parameter material in one annex.

I look forward  to any comments that you might have on this proposed material.
