Thanks for taking the time to look over my document. Sorry for the confusion - my intention was to focus on the parts that I was unsure about (and leave out details for the other parts). The next paragraph addresses the comments/questions that you embedded in the Word document.
I agree with your assumptions. Regarding credentials, I assume we will begin testing with credentials disabled, and then switch to "Credentials for Bind Only" after we have things working. Regarding extended diagnostics for the Start-Return: if we decide to add any extended diagnostics, I will be happy to support them.

Dear Tim,
I looked at your document and was a little confused.
I took the liberty to express it with my words and representation. I updated your note to detail each interaction (for the START and the TRANSFER-DATA). I have highlighted in yellow what is not clear to me from the prototype implementation.
Considering that you are implementing and in case the note is not clear or incorrect, we can discuss it on the telephone.
It may be useful to complement this note with the other operations and possibly the Association Control procedure operations to make sure we all have the same understanding.
