[Css-csts] Service and Procedure State Model

All,

Attached is a formal model of the service and procedure states. The
model is based on the discussions at the CCSDS Spring meeting, and
follows the state definitions in the draft Recommendations.

The aim of the model is to provide a more formal, UML 2.0 based
definition of the state transitions.
