Hello Colin,
I think the draft looks quite good. I added a few comments.
I had some confusing messages from Microsoft/Sharepoint indicating that it could not save my changes back into SharePoint because the file was in an old format (.doc). But it appeared to save the changes but it wasn't entirely clear that it had done so. So I saved it as .docx and the error messages went away. So you will find that there is a.doc file and a .docx for the SMAC white book. The link to the .docx file is:

My suggestion is that we just moved to the .docx file. As far as I know the need to stick with .doc files has essentially gone away with the new Secretariat staffing.

Section 3 of the document contains the diagram and write-up, the rest of the doc can safely be ignored for the time being.
If anybody is interested, I've pushed an update of the UML model containing the simple post pass report (SPPR), along with an auto generated schema for the SPPR to GitHub. This can be found at the following URL;

Hopefully the strawman SPPR will serve as a starting point for further discussions. It incorporates pretty much all the aspects of the non-NASA post pass reports and enhances this slightly. I deliberately did not try to address the complexity of the DSN report explicitly. I did however include a SPPREnhancedReport class which could be used to attached an enhanced report the format of which would be defined by an agency specific schema.
Comments and criticisms gratefully received as always.
