[cssm] PDF RID form for SMURF agency reviews

Dear CSSM Colleagues,

Please use one of the attached PDF RID forms for submitting RIDs against the SMURF book.  Agency reviews should be concluded by December 4.  The multi-rid form is more convenient for entering RIDs as it automatically propagates metadata such as book number, submitter name, etc.  The single-rid form can be used where local cybersecurity forbids the use of javascript.
