[Sls-mhdc] Revised draft of GB for 123.0-B-2

Dear all,

As per action item MHDCA-2005-6, I have uploaded the draft including my 
review of chapter 4.

I plan to run some simulations today regarding the comparison with a 
external quantizer + lossless, so that we can discuss the preliminary 
results on Thursday.
