[Sls-mhdc] Draft Green Book for CCSDS-123.0-B-2 uploaded

An updated draft is available:
	CWE Private / 123.1-B / GreenBook / 120x2g2-2018.10.10.{pdf,docx}

The bulk of the changes since the Oct 8 draft are in chapters 1-3. I think those chapters are essentially complete (or, perhaps more accurately, I think they're ready for someone else to review).
