[SLS-CC] TC coding Blue Book, version approved by WG for Agency Review

Dear Gian Paolo,

thanks to Ken's work, please find attached a new draft of the TC coding 
Blue Book, implementing the changes agreed with the WG and hopefully 
answering the questions
you raised in the past few months.
As a summary, this draft reverse the order of randomisation and encoding 
for LDPC and add support for USLP.
In order to simplify Tom's job, Ken did the following:

- All the intermediate change tracking has been reconciled
- The color code is as follows.  Black text is also unchanged from the 
currently published version of this book, and is included for context. 
 All changes are shown in blue, with strike-outs for deletions, and new 
text for additions.  There are just a couple notes to the editor in pink, 
when the changes may not be self-explanatory.  Sections of the currently 
published Blue Book that have no changes are omitted for conciseness.

Based on the consensus expressed by the WG, I would kindly ask you to 
initiate the process to start the Agency Review for this new version of 
the Book.
