[SIS-Voice] update of magenta book including the PIC

Dear WG
Here is an update of the Red Book, with all the comments that we get in Noordwijk and after via E-mail (thanks a lot for the inputs).
Also the PIC is incorporated, there are some sentences marked in yellow, that are the things that we need to discuss / agree, can be via E-mail or directly in London
Please review it and send your comments in the document or via E-mail.
