[Sis-ams] A couple more tweaks to the AMS Red Book

Season's greetings, AMS fans.  Tim has pointed out a couple of errors in the RAMS procedures (in 4.4.5 item 2 and 4.4.7.5), which I have corrected in the newest version of the spec that I uploaded to CWE a couple of minutes ago - "markup7".  Also, Pat uncovered some ambiguity in section 5.1.5.7, which I hope is now corrected.

More substantial than either of these, though, is an error in the structure of several MAMS "Supplementary Data" structures: when I was revising to change continuum number from 24 bits to 15 bits (because it has to be able to fit into a 16-bit subject as a negative number), I modified the RAMS message definitions but missed the Assertion and Cancellation structures for subscriptions and invitations.  I have now changed continuum number in all four of these guys (5.1.5.10, 5.1.5.11, 5.1.5.17, 5.1.5.18) from 24 bits to 15 bits plus one high-order filler bit for octet alignment.  Anybody see any problem with this?
