[Sea-time] Mission survey form is available

Hello,

To assist in collecting and processing data for the mission survey, 
Carlyn has prepared a Google Form:

This is publicly available, and you are all welcome to use it when you 
request inputs from your mission personnel. This may be easier than the 
previously suggested Word document. Please proceed in a way that works 
for you. A summary of mission responses so far can be seen at:

After the data is collected, we will have the data in spreadsheet form.
