[MOIMS] [CESG] Tables for CESG Report

Dear MOIMS leads,

For your report to MOIMS, please use the below template. It is the same 
one used previously, but we have noted that some WGs have started 
diverging from it, thus causing us unnecessary re-editing. Also, please 
make sure that tables are inserted so that they remain editable (i.e. do 
not use images).
