[Css-csts] updated MD-CSTS package on CWE

CSTSWG colleagues -

The zipped folder contains:

a)      The draft Red Book, which has been updated to include the reference to the SANA functional resource registry and associated cross-references, and modifications to section 7 to accommodate the removal of the BIND invocation's responder-port-identifier parameter as a configuration parameter of the Association Control procedure but still retained as a service management parameter of the MD-CSTS service itself.

b)      The set of original graphics used in the book. This is unchanged from the previous draft but included for completeness of the package.

c)       A text file of the ASN.1 modules. This is unchanged from the previous draft but included for completeness of the package.

To the best of my knowledge, this update resolves all remaining issues and is ready for submission to the Secretariat.
