[Css-csts] Effect of Production Status on START operation

Members of the CSTSWG ---
The extended START diagnostics 'unable to comply' and 'out of service'
indicate that the START invocation is to be rejected when the production
is interrupted or halted, respectively. However, Table 5-21 (in Annex B,
see below) states that the "other" confirmed operation invocations
(which includes START since it not otherwise named in the table) are be
accepted when the production is in either of these states. Which is
correct?

Also, Tables 5-20 and 5-21 are now in Annex B, and I found multiple
references to "table 5-21" in the text. The tables need to be relabeled
"Table B-1" and "Table B-2", and all references to them in the text need
to be re-referenced to Annex B.
