Good example to look at. Follows the ISO spec, uses their terminology, contains the kinds of columns and indicators required, and also provides an example of the kinds of header / identifying information that is required.
It's also quite concise, but still a non-trivial amount of work, since it compresses the references to a complex 75 page spec into 14 pages of tables with references to a number of standards, clear indication of what is mandatory, and check boxes for implementation conformance.
