[Smwg] Service Management Utilization Request Formats Book - White Book draft 0.01 (was Planning and Service Request Book)

Dear all,
                  I've just uploaded the revised draft of the Service 
Management Utilization Request Formats Book. This has been updated in line 
with the discussions in Darmstadt. I've also tried to address JPs need to 
be able to chain service packages. JP let me know if what is there is okay 
for you. If not I think we need to discuss this a bit more so I understand 
better what you need to do.
