As documented in file “MO M&C Services Input for Blue Compared to Red 3” 
There were major changes made to the Red-3 version of the document
that was sent out for Agency Review and the version that was subsequently
submitted for CESG and CMC approval to publish.

This violates CCSDS A02.1-Y-4, Clauses 6.2.5.a and 6.2.5.c:

6.2.5 Formal Agency Review
a)	if technical issues are identified in the course of a review, those issues must be resolved and the review must be repeated before approval can be sought for a change of document status; 

c)  if substantive changes are made to a document that has completed review without technical comment, the Secretariat shall conduct a final review in which Agencies can approve or reject the document but may not suggest additional changes; 

The document should be edited to reflect all of the requested changes that were provided during the CESG review, be given a thorough review by the WG and the MOIMS AD, and then sent out for another Agency Review.
